Important Documents
Any necessary documentation relating to your care and identification should be brought with you on the day you come to our center. A medical professional or one of our trained staff will be able to advise on the documentation you’ll need to bring.
Personal Belongings
Whether you're here as a day patient or you'll be hospitalized for several days, ensuring you bring personal belongings to suit your circumstances is always recommended. This could mean your phone, bag, and charger, or changes of clothes and personal hygiene items depending on how long you will be with us.

Medication List Form
A complete list of your medication is a requirement for your visit and greatly helps our team of medical professionals in providing you with the best possible care with far fewer risks in the process. List all medications you currently take, including doses. You can print off a copy of our form online today.
CaringBridge
CaringBridge is a community support non-profit network that offers care, planning, and empathetic services to aid in your recovery or long-term care. A virtual support planner helps family and loved ones coordinate care and tasks, providing an active community and open communication for enhanced support overall.
Hospitalist Program
Our specialist hospitalist teams are specifically trained to provide care and support for those that require care in a hospital setting. Through our support and coverage, we help reduce stays in the hospital, improve quality of life, and ensure your treatment plan is working for you. Our team works 24/7 to provide the care needed, so our patients never have to go it alone.
FAQs
Every area except for the ICU allows you to bring personal belongings with you. If you or a loved one is placed in the ICU, we will ask family or friends to take personal belongings home with them.
If you will be hospitalized during your time at Northwest, we'd recommend packing a small suitcase with your belongings. If at all possible, limit yourself to one bag.
If you lose your documentation or don't have it with you on entry, our team will work with you to ensure all critical documents are accounted for. However, you will need to ensure you bring a form of legitimate ID with you.
